Copa Health offers a range of community-based living options and support services for individuals requiring 24/7 supervision. Our programs empower individuals with disabilities to live as independently as possible while fostering community integration and personal growth.

IDD Community Living Services

Copa Health’s Community Living Services provide small group home environments (3-5 residents) for individuals with intellectual / developmental disabilities. These homes promote:

Group homes for adults are available in Arizona, offering a comfortable setting for individuals requiring daily living assistance.

Behavioral Health Community Living

Behavioral Health Community Living provides safe and secure housing for adults diagnosed with serious mental illness. This program helps our members maintain independence in their own homes. Services and supervision are provided by trained personnel and include:

IDD Community Support Services

This program helps children and adults with IDD maintain independence in their own homes by providing:

These services are tailored to individual needs, ensuring comfort and self-sufficiency. We provide IDD CSS services in Maricopa, Gila, Pinal, Coconino, Navajo counties.

Individually Designed Living Arrangements (IDLA)

Our IDLA program supports individuals with IDD living in their own homes, which they own or lease. Individuals handle their home costs, while Copa Health provides services like:

  • Personal care
  • Habilitation
  • Housekeeping

This program allows individuals to customize their living arrangements to suit their preferences and needs.

Elder Care Services

Our Elder Care program supports older adults in their own homes, helping them maintain dignity and independence. Services include:

  • Personal care
  • Housekeeping
  • Respite care

Copa Health is contracted by Area Agency on Aging (AAoA) and UnitedHealthcare (UHC) for Elder Care Services.

Supportive Housing and Permanent Employment

Copa’s Hope (Permanent Supportive Housing) and SHAPE (Supportive Housing And Permanent Employment) programs are designed to help individuals secure and maintain housing and employment.

Hope Permanent Supportive Housing (PSH) services provided:

  • Personal hygiene training and reminders
  • Personal safety and hazard skills training
  • Use of coping skills to replace problematic symptoms and behaviors
  • Communication skills
  • Financial management and budgeting assistance
  • Scheduling and appointment adherence
  • Identification of community resources and support systems
  • Health and Wellness
  • Case management/service coordination
  • Housekeeping skills training
  • Education surrounding meal planning and preparation
