Eligibility
Copa Health provides behavioral and mental health services to individuals with developmental, intellectual, or behavioral challenges in Arizona. We offer a wide range of services and programs that meet the physical, emotional, and social needs of each person we serve. To access our services, you need to meet certain eligibility criteria depending on the type and source of service you are seeking.
Eligibility Criteria
The eligibility criteria for our services vary depending on the type of service, the funding source and the referral process. Some of the general criteria include:
- Having a diagnosis of a developmental disability, a serious mental illness or a general mental health condition
- Having a referral from a qualified agency or professional, such as the Division of Developmental Disabilities (DDD), the Regional Behavioral Health Authority (RBHA), the Rehabilitation Services Administration (RSA) or a primary care provider
- Having an assessment of your needs, strengths, preferences and goals by one of our staff members
- Having a service plan that outlines the type, frequency and duration of services you will receive
- Having a funding source that covers the cost of services, such as Medicaid (AHCCCS), Medicare, private insurance or private pay
How to Apply
To apply for our services, you need to follow these steps:
Contact us by phone, email or online form to request information or schedule an appointment
Provide us with your personal information, such as your name, date of birth, address, phone number and insurance information
Provide us with your referral information, such as the name and contact information of the agency or professional who referred you and the reason for referral
Provide us with your consent to receive services and to share your information with other providers as needed
Complete an assessment with one of our staff members to determine your eligibility and service needs
Work with us to develop a service plan that matches your goals and preferences
Start receiving services according to your service plan
Insurances accepted
Copa Health accepts all insurances, including AHCCCS and private insurances; however this may be limited by location and/or services. Self Pay
Copa Health accepts self pay at certain locations. Contact the location at which you'd like to receive services.
